It's a phrase heard all too much in baseball, and one that was proven true yet again in Philadelphia on Wednesday night.
After two ninth inning errors by the Rockies allowed the Phillies to stay alive, Chase Utley singled home the game-tying run and was followed by walk-off home run by Ryan Howard, as the Phillies won 6-3.

The Rockies opened the scoring in the third, when Justin Morneau led off the inning with a home run. Wilin Rosario would double home another run later in the inning, as the Rockies took a 2-0 lead.
The Phillies would get one back in the bottom of the frame, courtesy of an RBI single by Howard before tying the game at 2-2 on a Marlon Byrd single in the fifth.
A shaky Phillies bullpen would strike again in the eighth, however, as Jake Diekman allowed a solo home run to DJ LeMathieu to put the Rockies up 3-2.
It would be the Rockies that collapsed completely, however, as errors allowed the Phillies to steal the win.
After Cesar Hernandez was retired to start the ninth, Tony Gwynn Jr. reached base on a throwing error. Ben Revere would follow that with a single to put the tying run in scoring position before Jimmy Rollins lined out to put the Phillies down to their final out.
With just one out remaining, Chase Utley stepped to the plate. Utley would work a 3-0 count before driving the fourth pitch deep, but foul. On the very next pitch, Utley would single to right to tie the game.
Ryan Howard would be next, and would play hero on the night, as the veteran first baseman crushed the sixth pitch of his at-bat to left center for a three-run home run to give the Phillies a 6-3 win.
Next up for the Phillies is a five-game home series against the Mets. David Buchanan will make the start Thursday night for the Phillies, squaring off with Zack Wheeler.
